Historically, SAT1 circulation was largely limited to sub-Saharan Africa, where it persisted in endemic zones, particularly in East and Southern Africa. Occasional detections outside this region were rare and short-lived, with no sustained transmission.

That pattern has shifted dramatically. Since 2023, SAT1 has been detected in countries including Qatar and Jordan, followed by rapid expansion in 2025 into Iraq, Kuwait, Türkiye, and Egypt. By 2026, the virus had reached Azerbaijan, Lebanon, Israel, Cyprus, and Greece—marking a notable spread into Europe.

This transition signals a move from sporadic incursions to sustained regional circulation across the Eastern Mediterranean, West Asia, and North Africa.

Gaps in Vaccination and Surveillance

A key challenge in controlling SAT1 is the lack of cross-protection from existing vaccines. Most vaccination programs in affected regions target serotypes O, A, and Asia-1, leaving livestock populations susceptible to SAT1 infection.

Limited availability of SAT1-specific vaccines, combined with gaps in surveillance and inconsistent veterinary infrastructure, has created conditions for undetected transmission and rapid spread.

Drivers Behind the Expansion

Several factors are contributing to the emergence and spread of SAT1:

  • Informal cross-border livestock movement, especially among small ruminants
  • Seasonal increases in animal movement tied to cultural and religious events
  • Environmental pressures such as drought and land-use changes
  • Increased interaction between wildlife reservoirs and domestic livestock

While improved surveillance has aided detection, the scale and speed of spread suggest a true epidemiological shift rather than simply better reporting.

Implications for Global and U.S. Preparedness

The growing geographic footprint of SAT1 increases the complexity of global FMD risk. For FMD-free countries like the United States, the emergence of a new serotype in multiple regions introduces additional uncertainty in prevention and response planning.

Current vaccine stockpiles may not include well-matched SAT1 strains, and the expanding distribution raises the risk of introduction through travel, contaminated products, or fomites.
